Summary
The multi-year research effort in this volume has demonstrated that (1) similar challenges are faced by all SERIDAS rivers, (2) problem recognition varies widely across basins, and (3) response strategies are not sufficiently grounded in the long-term goal of river sustainability. We recommend that river managers and stakeholders follow these guidelines: Determine the river’s dependable yield – the water supply, averaged over the period of the most severe drought experienced in the historical record. Use the dependable yield to define and implement a river management strategy that carefully balances human and ecological needs in the basin. To meet human needs, do more with less – practice water conservation. This is the best obtainable strategy for redefining and maintaining river sustainability.

Summary
The Rio Grande marks the border between the United States and Mexico, which makes its management particularly complex. A century of engineering and binational governance has shaped the region; today there are multiple reservoirs, diversion channels, and irrigation canals. International and national river managers consider challenges from reservoir sedimentation, population growth, and land use change. Climate change, on the other hand, is barely addressed. Managers must plan for a Rio Grande that by mid-century will have lost 40–50 percent of current water levels. Urban population will have doubled. Irrigated agriculture and cities will continue to use the bulk of available water. The most promising response to meet human and ecological water needs with reduced dependable yield is conservation. To address water scarcity, IBWC/CILA should develop a sustainability plan that balances reduced dependable yield with human and ecological needs. Progress should be reviewed and necessary changes to water allocations be negotiated at 5-year intervals. Irrigation districts, municipal utilities, and environmental stakeholders should work with IBWC/CILA to develop drought management for their sub-basins.

Summary
A reservoir is a replacement of a segment of the river with a watercourse that is quite different, a larger, more quiescent water body with different water quality and capable of stratification. This chapter describes design and functions of river reservoirs. The main challenges faced by SERIDAS reservoirs are highlighted. Special attention is given to a reservoir’s dependable yield, which can be maintained constantly without failure throughout the time history of reservoir storage. All reservoirs act as sediment traps and will eventually silt up unless special actions are taken to manage sediment. Reservoirs significantly alter the hydrology of the river downstream from the dam and can affect its water quality and its ecosystem. With increases in population and agriculture, water demands will increase, exacerbated by climate changes. Skillful management will be needed to temper conflicts that arise over how reservoirs will be operated under situations of incompatible objectives.

The records of nine patients with double-chambered right ventricle, ventricular septal defect and subaortic narrowing were reviewed to clarify the natural history of the components of this pathological combination. All nine patients presented with heart failure related to a perimembranous ventricular septal defect, which underwent spontaneous reduction in eight and closed in one. Echocardiographic diagnoses of double-chambered right ventricle and subaortic narrowing were delayed in eight of the nine cases. The ratio of the distances from the pulmonary valve to moderator band/tricuspid valve annulus (the displacement index) was less than one in all patients. Subaortic narrowing was always discrete and was present at the diagnosis of double-chambered right ventricle. The subaortic narrowing was always recognized echocardiographically, and was seen in eight of nine patients at angiography. The subaortic gradient was 0–20 mm Hg and the subpulmonary gradient 0–110 mm Hg at catheterization. Five patients demonstrated aortic regurgitation. Subaortic narrowing should be diligently sought among patients with double-chambered right ventricle, and vice versa, even in the absence of a ventricular septal defect which may have closed. Both the subaortic narrowing and subpulmonary obstruction may progress, but probably at different rates. Patients with a perimembranous ventricular septal defect, particularly with a displacement index of less than one, should be monitored carefully for evolving subpulmonary obstruction, subaortic narrowing and aortic regurgitation. The subpulmonary obstruction should be resected at the time of any other surgical intervention. The indications for resection of the subaortic narrowing remain ill-defined.

Summary
Introduction
One key to measuring the body composition of an animal without killing it in the process is the fact that water is not evenly distributed in body tissues. Fat contains substantially less water than lean tissue and this difference means that the fatter an organism becomes, the lower the water content as a percentage of its total body mass. Since body mass is relatively easily measured, if the total water content of an animal could also be quantified, a method would be available for estimating fatness. Several of the methods detailed in this book (for example, TOBEC: Chapter 5 and BIA: Chapter 6) rely on the differential water contents of lean and fat tissue to quantify body composition. Dilution methods are also based on this principle. Initial attempts to measure the body water by dilution used compounds that were soluble in water (such as antipyrene: e.g. Soberman et al., 1949, urea: e.g. Meissner, 1976 or thiocyanate: e.g. Hollander et al., 1949). The discovery of isotopes of oxygen and hydrogen in the 1920s and 1930s had opened up the opportunity of using these materials to ‘label’ the body water directly, and the first attempts to do this were made in the 1930s (von Hevesy & Hofer, 1934). The isotope dilution method grew out of these initial studies.
Theory
To understand how the isotope dilution method works, it is perhaps useful to consider an analogous situation with which many animal ecologists will be familiar: the problem of determining the size of a population of animals that live in a given area.

The Infrared Astronomical Satellite (IRAS) has detected many galaxies in the infrared (IR), most of which have fairly steep 25μ to 60μ spectra. Many quasars and active galaxies exhibit a significantly flatter spectrum in the infrared. Several studies, for example, DeGrijp et al. (1985) used this characteristic to select a subsample of “warm” objects from the IRAS PSC (1985).

Two cases with laryngeal manifestations of systemic lupus erythematosus presented demonstrate the value of serial indirect laryngeal photography and results of treatment for the mucosal and serosal effects of the disease.
Systemic lupus erythematosus is an autoimmune collagen vascular disease which produces widespread damage to connective tissue, blood vessels, serosal surfaces and mucous membranes (Rodman et ah, 1973). It is no surprise that with such a widespread disorder the upper airways too may show involvement.Estimates of the number of patients with laryngeal involvement have ranged from one percent by Dubois (1966) to one-third of patients by other authors (Babich and Tranaov, 1970; Minchina et al., 1971).
Systemic lupus erythematosus may cause laryngeal symptoms referrable to an active mucosal, submucosal or serosal process. With acute flares of the disease, mucosal ulceration, edema, and submucosal hematomas may result in hoarseness and throat pain (Scarpelli et al., 1959). One case of epiglottitis caused by increased mucosal swelling has been reported (Toomey et al., 1974). As the disease progresses, late effects of mucosal disease including corditis, mucosal thickening, laryngeal scarring with stenosis and laryngitis with dry, thickened vocal cords may appear (Babich and Tramaov, 1970; Smith and Ferguson, 1976). With serosal involvement, the patient may demonstrate perichrondritis, cricoarytenoid arthritis or actual vocal cord paralysis (Dubois, 1966; Montgomery and Lofgren, 1963).
Histopathologic study of involved mucosa along with cultures of the tissue is not diagnostic of systemic lupus erythematosus but can help to rule out other disorders similar in appearance such as tuberculosis and rhinoscleroma. Klebsiella rhinoscleromata infections particularly can cause a similar early inflammatory mucosal and late scarred picture.
Microscopic sections of mucosa from a patient who died of laryngeal edema secondary to systemic lupus erythematosus appeared similar to mucosa from a patient with subglottic stenosis (Smith and Ferguson, 1976) with an infiltration of histiocytes, lymphocytes, plasma and mast cells.
The following two cases demonstrate the differences in the course of serosal and mucosal airways disease.
